Q: What is concierge medicine?


A: Concierge medicine, boutique medicine, and direct practice are all terms used to describe a new style of medical care in which patients pay an annual fee to the doctor for enhanced medical services not normally covered by private insurance.



Q: How does a Hybrid Concierge medical practice differ from a traditional Concierge Medicine?


A: In the Hybrid Concierge model which I am offering to our patients you are responsible for an annual fee. Additionally your insurance is still billed for normally covered services rendered by my practice. This is the greatest difference between the Hybrid
Concierge model and the straight concierge model which does not bill private insurance at all and the membership fee is significantly higher.



Q: Do I still need to keep my health insurance?


A: Yes, your insurance will still be billed for normally covered services rendered by Dr Matt and other staff members as needed. You will also need insurance outside the our office for ER visits, hospitalizations, specialists, etc..



Q: Are you limiting the number of patients who can become members?


A: Yes, due to the nature of the Hybrid Concierge practice, where Dr Matt plans to devote more time and attention to each patient who joins, membership will be limited and offered to patients on a first- come first-serve basis. This way, we can ensure each patient who becomes a member will receive the highest level of attention and care possible. Priority will be given to existing patients.



Q: How much is the Annual Physician Fee to become a member of Dr. Matt's Hybrid Concierge?


A: $2000 per patient regardless of sex, age, or medical preconditions. The fee must be paid annually and in full.



Q: When will the Hybrid Concierge practice begin?


A: June 1, 2017



Q: If I become a member of the Hybrid Concierge, how often am I allowed to see my doctor?


A: There are no limits on the number of times that you can be seen for medical conditions.



Q: Can I remain a patient of Dr. Matt's if I am unable to pay the Annual Physician Fee?


A: Although we have done our best to make this affordable, We understand some of our patients will be unable to afford the Annual Physician Fee. If that is the case, we welcome you to remain a patient of AllCare Medical Centers and see the other great providers here.



Q: Do I still have to pay the Annual Physician Fee if I am on Medicare?


A: Yes, you will be required to pay the annual fee. We will bill Medicare for medical services rendered.



Q: Will I receive a discount if I don't have my annual physical exam or use all your services?


A: No. Your annual fee covers my availability for complete supervision of your medical care.




Q: If I become ill and require extensive medical services, will my Annual Physician Fee increase?


A: No



Q: Will there be any problem for outside tests ordered by Dr. Matt?


A: No, as long as he is listed on your insurance plan as your PCP.


 

Q: What if Dr. Matt is out of town or on vacation?


A: If Dr Matt is not available due to illness and/or vacation, members of the Hybrid Concierge plan will have care provided to them by another provider within the AllCare providers group.
